"FOREVER" QUILT SQUARE  $20                                                                                                                
 
To participate in ANA's "Forever" quilt, to be displayed at future ANA events, please select the number of quilt squares you wish to purchase. Decorate the material in memory of your loved one and return it to ANA for inclusion in the quilt. "Forever" quilt squares are available for a minimum $20 donation each.

MEMORIAL SIGN  $10
 
 
Memorial Signs in memory of children being remembered at the walk are available for a minimum $10 donation and will be displayed at the walk in the Columbia Pavilion. The 8.5" x 11" signs are personalized with your loved one's name. Take the sign with you on the walk or before you leave for the day, and keep it as a memento (pictured at right, 2010 Memorial Sign).

#2  CALCULATE YOUR MINIMUM DONATION AMOUNT
 
Multiply the number of items you purchased by the minimum donation requested for that item (i.e. 2 memorial signs at $10 ea. = $20 total)
If you have ordered different items (i.e. 2 shirts and 1 memorial sign), remember to add the item totals together.
 
Merchandise will be available for your pick up at ANA's annual October Memorial Walk. For shipping charges, contact Michelle at mgmosca@msn.com before payment is made.
